In-Home Supportive Services and Adult Protective Services (IHSS/APS)--In the IHSS program, Public Health Nurses work in collaboration with social workers (SWs) to assist with preventing out-of-home care placement of elders and dependent adults through the assessment of need for in-home support services. PHNs collaborate with SWs on elder abuse cases and provide health assessments and assistance with accessing medical services.
